1098-T Form Sign-In Information

The 1098-T form is the Tuition Statement that your college or career school uses to report qualified tuition and related education expenses to you and the IRS. You or your parent/guardian may be able to claim these expenses as education-related tax credits.

Click here to view or print your 1098-T form online via the Tab Service Company (TSC) web services site.

Use the following information to sign in:

  • Site ID: 11558

  • Username: Your UCLA Extension ID (starts with X)

  • Password: For your initial login, your password is the last 4 digits of your Tax ID number/Social Security number 

    • If you do not have or did not provide your Tax ID number/Social Security number to us, your default password will be 0000
